Application Information

• An unsigned individual colour photo of each person attending the appointment – this should be a passport sized photo taken within the last month;
• The DNA offer letter you received from HM Passport Office;
• A copy of your payment receipt;
• A printed copy of this appointment confirmation email;
• A government approved photo identification document, such as a passport, driving license or ID card (with English Translation of the identification document, if needed) along with a colour photocopy of that document for each person attending the appointment;

If you do not bring these documents, your appointment will not go ahead, and you will have to make another appointment.

If you have sent your original identity document to HM Passport Office, we will either send a copy of it to the supplier or you can request it is returned to you so that you can bring it to the appointment with you.
